To facilitate the approval process, please send the following supporting documents for our review in one of the following ways: Act Now Fax to us at (852) 2968 1327, or mail us to Citibank (Hong Kong) Limited, GPO Box 3281, Hong Kong. Documents required Please write your name and Hong Kong Identity Card number on all the copies of the documents Copy of your HKID and with your signature on blank area Proof of your residential address no more than 1 month old, e.g. electricity bill or bank statement showing your name Personal Data Opt Out Form Any one of the following proofs showing your name (a) For standard Credit Card applicants: Latest 1 month's payroll slip OR FULL SET of Bank statement/ passbook showing your name, account no. and latest 3 month's salaries OR Other asset proof, e.g. current full month fixed deposit advice (b) For student Credit Card applicants: Front and back copies of student identity card (please refer to identity card photocopying hints) Hints for photocopying/ faxing Identity Card copy A clear Identity Card copy ensures quicker processing Enlarge to 200% For the best result, please choose a lighter color mode when you make photocopy on HKID card Sign and write your name and Identity Card number on blank area If self-employed, please also submit: Personal Tax Assessment and/or Profit Tax Record, AND Other asset proof, e.g. current full month fixed deposit advice If not a Permanent Hong Kong Resident, please also submit: Employment contract/ employment letter showing employment period AND Copies of HKID and passport showing the name, passport number and nationality of the applicant. If you have a Former Name, please also submit: Documented proof, e.g. Deed Poll If you have a relative/ spouse that is an Employee/ Director of Citibank (Hong Kong) Limited, please also provide: Name and Department of that employee/ Director.
