The issue of any Citibank Credit Card / Citibank Private Label Card / Diners Club Card to me will be accompanied by the relevant Citibank Credit Card / Citibank Private Label Card / Diners Club Card Agreement / the Conditions of Issue of Octopus Automatic Add Value Agreement issued by Octopus Cards Limited ("OCL") / the Terms and Conditions for Octopus Rewards Programme issued by Octopus Rewards Limited ("ORL"), whichever is applicable (each an "Agreement", as amended from time to time), copies of which are also available upon request and the Agreements provided by OCL and / or ORL can be obtained from the Octopus website, www.octopus.com.hk.
I may from time to time apply for further Citibank Credit Cards / Citibank Private Label Cards from Citibank or Diners Club Cards from Diners and Citibank / Diners may issue the same to me without a written acknowledgement of the relevant Agreement(s).
I agree that all Terms and Conditions of the Agreements, the Policy Statement relating to the Personal Data (Privacy) Ordinance ("Policy Statement") and this form will constitute my agreement with Citibank / Diners for the application and the use of such Citibank Credit Card / Citibank Private Label Card / Diners Club Card.
If you do not wish Citibank to provide your personal data to Octopus Cards
Limited/Octopus Rewards Limited regarding Octopus Citibank Platinum Card/Octopus Citibank Gold Card, please indicate your preference in the section of "Opt-out

Perpetual annual fee waiver is exclusive for Citigold customers who hold a Principal Citibank Credit Card and maintain the "Average Daily Combined Balance" of HK$1,000,000 or above. The "Average Daily Combined Balance" is calculated based on the sum of your daily balances of deposit, investment and accumulated paid premium of selected insurance products of all your single name account(s) and joint name account(s), divided by the number of days in the month. Selected insurance products mean insurance policy(ies) with savings element or investment-linked insurance. For insurance policy(ies) with savings element, accumulated paid premium means total premium paid minus value of outstanding policy loan. For investment–linked insurance policy(ies), accumulated paid premium means total premium paid minus value of fund sold. Should the "Average Daily Combined Balance" fall below the minimum balance requirement for 3 months, a monthly service fee of HK$300 will be charged (the monthly service fee for International Personal Bank Hong Kong customers is HK$500 or equivalent) else no annual fee applies. For the fees of individual service, please refer to the latest service fee guide. For account opening enquiry, please call 24-hour Citigold dedicated hotline at 2860 0111.If customers close his/ her Citigold account after card approval, Citibank reserve the right to charge an annual fee for each Citibank Credit Card without prior notice. I acknowledge receipt of the Citibank Credit Card / Diners Club Card Fees Schedule, the Major Terms and Conditions of Citibank Credit Card Cardholder Agreement / Diners Club Card Cardmember Agreement and the Policy Statement.
I understand that Citibank / Diners will review my credit report from the Credit Reference Agency in assessing the application.
If I currently hold an Octopus and I am applying for an Octopus Citibank Platinum Card/Octopus Citibank Gold Card above, my existing Octopus with its associated services will NOT be transferred or cancelled upon approval of this application. A new Octopus (with a new Octopus ID) will be embedded in the Card once this application has been approved. The total outstanding balance, security access and Octopus Reward$ earned in my existing Octopus will NOT be transferred to my new Octopus created under this application. If needed, I will need to contact the relevant service provider.
I understand that all information obtained by Citibank / Diners during the credit card application process is necessary (unless otherwise specified as optional) for Citibank / Diners to process my application and if I fail to provide the same to Citibank / Diners, Citibank / Diners will not be able to issue any card or provide any services to me. I agree that my personal data and information collected by Citibank / Diners from time to time may be used and disclosed for such purpose and to such persons (whether in or outside Hong Kong) as set out in the Policy Statement.
With respect to this form and any other relevant documents, in case of any discrepancy between the English and Chinese version, the English version shall prevail. |

Privacy
